There's really no reason why a starter shouldn't come out if indeed all the bolts are out. So if late in your season, if your starters are getting subbed out for the entire game and don't ever come back, it was happening to me, but after awhile I found the solution. Go to Franchise Staff, then Auto Subs, and turn all the sliders to 0. Its very annoying and takes awhile, but trust me, it works.
